Schedule a Flu Shot !
Proact will be available for flu shots:
Wednesday, November 1st 11am – 2:00pm Civic Center Basement Level
Friday, November 3rd 7 am – 9 am WEP Hiawatha Boulevard ***
Wednesday, November 15th 11:00am – 2:00pm Civic Center Basement Level
You must pre-register for your flu shot by visiting: https://kinneydrugs.as.me/ProactCivicCenterOpenEnroll.
Please arrive at your scheduled time and make sure to bring your OnPoint ID card at the time of your appointment. You or you covered family members may also get your flu shot at any local Kinney Drugs Pharmacy. Please call the pharmacy before your visit to make sure a pharmacist will be available to give the shot.
You must complete this Flu Clinic Consent Form and bring it with you.
